Signed Card in Match Box was a Paul Harris trick marketed by Gem and Jana Productions the same year it was published as Flash-Fold in The Magic of Paul Harris(1976).

You make the volunteer a member of the Society of Magicians Assistants and his initialed membership card ends up, folded in a regular match box.
